To create a Palliative Care Portfolio Entry:
- Log in to medportal
- Navigate to Postrgrad.medportal.ca and click on Family medicine from the Programs drop down menu
- On the left hand menu, click on Academic Program and then Portfolio
- Click on New Entry
- Fill in all of the required fields detailing your encounter along with the written Narrative
- When finished, click on Submit Entry
